Dementia Resource Community works with Age UK Wirral in supporting Wirral Memory Assessment Service. Our Memory Assessment Nurses are skilled Registered Mental Health Nurses with significant experience in assessing cognition. Our nurses carry out comprehensive cognitive and mental health assessments, to support NHS colleagues in the diagnosis of dementia. This process also highlights where memory difficulties are not due to dementia but other factors, which can be discussed and explained.
Those seen by our service receive information and support plans so that the processes are clear and that each individual seen has a high quality assessment.
Our Nurses will ensure anyone referred to us will receive a person-centred assessment. This is not a one size fits all process and will vary depending on presentation of the individual. It’s important to remember that a diagnosis of Dementia does not mean the end of a fulfilling life. With the right support and care, many people with Dementia are able to continue to live well and enjoy meaningful activities and relationships.
Age UK Wirral provides additional support to families which our nurse will advise on following assessment.
