Chapel House Care

Chapel House Care Commissions Dementia resource Community to provide two Admiral Nurses based at Chapel House Nursing Home and Plessington Court.

 

Our nurses provide support to the Nursing and Care Teams in terms of caching, education and case reviewing processes to ensure that evidence based practice is at the forefront of the care delivered.

Their role is also to support families often during times of transition this can be during the admission process, during a period of complexity and during end of life care. The focus of the Admiral Nurse is to work alongside families to offer expert practical and emotional support to help them cope with the difficult everyday reality of dementia and planning for the future.

 

Alongside supporting families directly, the Admiral Nurses will use their expertise and experience to support the best practice of other health and social care professionals in planning, coordinating, and delivering holistic dementia care.

 

Our nurses will can provide community appointments for those considering 24 hour care at Chapel House.

We are delighted to offer free, confidential half-hour appointments with a specialist Admiral Nurse at the GIFT Cafe in Puddington. This is a wonderful opportunity for local community members who are caring for someone living with dementia to receive expert advice and support.

 

What is an Admiral Nurse?

 

Admiral Nurses are specialist dementia nurses who provide compassionate, expert support and guidance to families and carers affected by dementia. They understand the unique challenges and emotional toll that caring for someone with dementia can bring.

 

Admiral Nurses can help you with:

 

Understanding dementia: Providing clear information about different types of dementia, symptoms, and progression.
Practical advice and strategies: Offering tips and techniques for managing challenging behaviours, communication difficulties, and daily care.
Emotional support: Providing a listening ear and helping you cope with the emotional impact of caring for someone with dementia.
Connecting you with services: Signposting you to relevant local resources, support groups, and other healthcare professionals.
Navigating the complexities of dementia care: Helping you understand the health and social care systems and access the support you need.
Promoting the rights of people with dementia and their carers.
This drop-in clinic at the GIFT Cafe offers a relaxed and accessible environment to speak with an Admiral Nurse. These appointments are completely free for members of the local Puddington community.
