Introducing the new Dementia Specialist Nurse Service

in Cheshire West

We are delighted to tell you that Dementia Resource Community (DRC) has been commissioned by Cheshire and Merseyside ICB & Cheshire and Wirral NHS Partnership to provide you and your patients with clinical support. Our service is to target health matters linked to significant escalations in symptoms of dementia.

Dementia care is a growing concern for families in Cheshire West, often leaving them feeling overwhelmed and under-supported. Our service addresses this pressing issue by providing three dedicated Dementia Specialist Nurses, each with extensive experience in managing complex dementia symptoms and collaborating within multidisciplinary teams. By offering personalised support and guidance to both patients and their families, we aim to enhance the quality of care and improve overall well-being. Our aims and objectives are:

To work to avoid hospital admission across Cheshire West.

To provide face to face support in helping people with clinical difficulties which are impacting on their dementia wellbeing.

To evidence any service gaps in the locality.

We will be working to support the urgent care team as a step down measure when there has been an acute need to avoid further crisis.

Our aim is to contact people referred within 72 hours where possible to complete an initial risk assessment process; this will dictate the priority of nurse appointments.

To re-direct patients to the Dementia Care Navigators or other suitable community based support once their clinical issue is on the pathway to being resolved.

This service will work in partnership with the excellent care offered by NHS colleagues. If you have any questions or concerns about the type and level of integration we offer.
